All participants are kindly requested to register in advance. Pre-registration is available by completing on-line registration on our website, www.APOAsp.org. In case you do not have any internet access, please contact the secretariat.

¡Ø Please be informed that the registration fees have been changed slightly compared to the ones in   1st Announcement

- Member means those who have a valid APOA membership.
- Accompanying person is limited to only the participant¡¯s spouse and children.
- Registration fees include:
  Participant - All sessions, Exhibition, Opening Ceremony & Welcome Reception, Presidential   Banquet, Congress bag, Any related documentation, Lunches and Coffee breaks
  Accompanying person - Exhibition, Opening Ceremony & Welcome Reception, Presidential   Banquet, Lunches and Coffee breaks

Notes
- Pre-registration will only be accepted until May 10, 2008.
- A confirmation letter will be sent within two weeks via e-mail upon receipt of your registration form   and full payment. If you do not receive the confirmation letter, please contact the secretariat.
- Modification or update can be made after log in with your User ID(E-mail) and Password by May 10,   2008. Registration items you can modify online are limited to personal information(except name and   e-mail). If you would like to modify the other items, please send your request to the secretariat via e-  mail.

Payment
- Payment can be made with Credit card only. It you have to pay with other methods, please contact   the secretariat.
- Use of international credit card such as VISA, Master, Diners and JCB is available except AMEX.

Cancellation & Refund Policy
- Cancellation received by May 10: 70% refund
- Cancellation received after May 10: No refund
- Cancellation requests must be made in writing to the secretariat via e-mail or fax.
- All bank charges for remittance will be deducted from the amount of refunds.
- All approved refunds will be processed after the congress.
